Employees injured on-the-job should be directed to one of the AlaMed industrial medicine physicians. Serious injuries or emergency situations should seek the most immediate and appropriate emergency care.

Call AVIZENT as soon as you are made aware of an accident/injury. Let the person who answers the phone know that you have a new claim to report and you will be connected with someone who can take the required information. Should you have a serious accident on the weekend or after hours, please call Avizent's toll free number, 1-800-277-7500 and you will be given instructions to talk to an adjuster to report the claim.

Make sure either your primary care physician has a supply of drug screening kits or send one from your office or job site to the doctor or hospital with the injured employee. If you do not have drug screening kits, contact Employment Sreening Services at 205-879-0143 or call WorkersFirst at 205-981-0086.

If the injured employee requires prescription medication, give them a ScripNet flyer to take to the pharmacy with them. They will be mailed a ScripNet Card for use within the pharmacy Network.

If the initial treating physician or hospital requests approval for referral to another facility, please have them contact Margie Dodson at Avizent for approval and she will direct them within the AlaMed Network.
